摘要
顶点(vertex) 是比昂(Wilfred Bion)用来指称观察所由之出发的立足点/视角的术语。其核心主张是:任何心理现象都只能从某个顶点被看见,因此改变顶点即改变所见——不是看见同一个东西的另一面,而是看见另一个东西。
这一概念在贝蒂·约瑟夫(Betty Joseph)手中被发展为一项临床工具。她指出分析师与病人各自持有一个顶点:分析师持有「他所希望达成的心理改变之理论」,病人则「意识上希望改变,却畏惧其平衡感受到任何扰动」——而后者才是病人的顶点。治疗任务因此不是替换病人的顶点,而是让两个顶点相遇,同时不让病人失去与其经验性自体的接触。
由此产生约瑟夫最具标志性的临床动作:把注意力从诠释的内容移到诠释被如何接收、使用或中和。同一份临床材料,从「内容顶点」看与从「过程顶点」看,会得出完全不同的结论。

一、比昂的定义:顶点是「每个人所认为最佳的组织」
Govrin 引比昂 1970 年的原句:
As Bion (1970) describes: “the psycho-analyst and the analysand each have a vertex (or vertices) which, if it were known, would indicate the organization each regarded as best” (p. 127).

这句话里最容易被忽略的是复数的可能性——vertices。一个人可以有多个顶点,而顶点之间的切换,正是分析过程中发生的事。
同时要注意,顶点的定义项不是「观点」而是组织(organization):顶点标示的是一个人把自己的心理生活组织成什么样,以及他认为哪一种组织是最好的。
二、约瑟夫的双顶点:两个顶点,两种「最佳」
约瑟夫把比昂的定义落实为一对临床可操作的规定。
分析师一侧的顶点:

四、临床材料:Mr. A. 与《致命的自负》——同一份材料的两种读法
(一)材料
Mr A was coming down a spiral staircase, carrying a pile of books. He had to hurry to go to a meeting and so put a book down on each step. Afterwards he went back to collect them, but found that one was missing, and felt sure that it had been taken. It was by H, expensive and important to him.

(二)内容顶点会怎么看
